Insurance and Billing
Arizona Ophthalmic Outpatient Surgery accepts most major insurance. We will bill your insurance carrier for you; however, we cannot tell you what your policy will cover. Since your insurance policy and coverage are between you and your carrier, we urge you to contact them prior to your procedure. You will be responsible for any co-pays and deductibles.

My first time in this clinic 10.01.24. Everyone in front office check in are very nice. I'm quite nervous about my procedure because it's my eye. I went through lots of procedure this one makes me anxious. My attending doctor Dr. J. Gaitan and Dr. Nieva both are knowledgeable and very nice gives me an assurance everything would be just fine. I didn't feel anything and I'm done. The clinic is very clean, well maintained. They have a good teamwork, helping each other. I'm very pleased the outcome of my procedure, now just the healing process. I'm recommending this clinic to my friends, family and anybody looking for a clinic and an opthalmologist specialist. Thank you everyone keep up the good work.
